Home Care Package

Home Care Packages can be an option if you need a coordinated approach to the delivery of your help at home – perhaps because you need help with many everyday tasks, or the care you need is more complex or intensive.

At Freedomlink Support Services we provide a Home Care Package (HCP) which is a more intensive, long-term package of aged care services to help people aged 65 years and over with higher care needs. These packages are ‘Consumer Directed’, meaning that you and the person you care for have a say in what is included in the package and how support is delivered.

To check your eligibility an assessment is carried out to evaluate your needs. You must be 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people.

Whether it's keeping in touch with friends, socializing or scheduling appointments, we can help you. We can connect you to your current network or help you build one. There are many options available to you such as: E.g. group hikes, individual care, kites (social group outings) and day clubs.
